PROTECT YOURSELF & YOUR FAMILY

1. If you're the accident victim, stay calm, assess your injuries, call 911 or yell for help from those nearby as needed.

2. If you are a family member or witness to a home emergency/accident or discover the accident victim, stay calm and do the following:

(a). If the victim is conscious:
talk to victim, assess injuries, call 911 as necessary. Victim will inform you if they have a Quickfor

(b). If the victim is unconscious or not able to talk, stay calm, quickly assess injuries, call 911, assist if you can -comfort, cpr etc. until help arrives.

(c) When Emergency Personnel arrive, inform them of victim's Quickform as it may contain vital information that will help them make a more informed decision regarding initial emergency care.

3. In the Emergency Room, be sure (that you or family members) present the patient's Quickform as it may contain additional information to help
with continued care.
